What happens after you submit a referral?
We've designed our intake process to be welcoming, transparent, and stress-free.
Step 1 — Submit Your Referral
Complete our online referral form with as much information as you feel comfortable providing.
Step 2 — We'll get in touch
One of our friendly Intake Team members will contact you to introduce ourselves, answer your questions, and learn more about your goals and support needs.
Step 3 — Getting to Know You
Together we'll discuss:
• Your goals and aspirations.
• The supports you're looking for.
• Your interests and lifestyle.
• Your preferred support times.
• Your NDIS plan and funding (if applicable).
• Any questions you may have.
Step 4 — Matching You with the Right Team
Finding the right support worker is important.
We'll carefully match you with staff who suit your personality, interests, communication style, and support needs because we believe great relationships create great outcomes.
Step 5 — Welcome to Pride
Once everything is in place, your supports can begin.
From day one, you'll have a team beside you that's committed to helping you achieve your goals and build the life you choose.

Through Support Without Barriers – Virtually Pride, participants can access engaging online supports from Regional Victoria and across Australia, helping remove barriers created by distance, transport, health, or location.
